I, THE PARENT/GUARDIAN DECLARE THAT MY CHILD PARTICIPATING IN CORINTHIANS SOCCER ACADEMY LLC ACTIVITIES HAS MEDICAL INSURANCE
REGISTRATION AGREEMENT
I, as the parent / guardian of an underage athlete, agree that the registrant will accept and recognize the rules of CORINTHIANS SOCCER ACADEMY LLC, its affiliates, organizations, and sponsors, mandatory for CORINTHIANS SOCCER ACADEMY LLC to the registrant in their soccer programs and activities.
I accept and acknowledge the possibility of physical injuries associated with soccer, thereby releasing, and discharging any liability to CORINTHIANS SOCCER ACADEMY LLC, its affiliated organizations, sponsors, employees, and associated personnel.
I give permission for my child to participate in CORINTHIANS SOCCER ACADEMY LLC, and I assume all risks and dangers inherent in such participation, and I hereby release CORINTHIANS SOCCER ACADEMY LLC, its employees and agents from all liability resulting from injuries sustained by my child during their participation in the activities of the Academy. I also authorize CORINTHIANS SOCCER ACADEMY LLC to make decisions regarding any medical care for injuries that affect my child. In addition, I extend this authorization for use in any medical or hospital facility to accept this document as authorization to provide emergency care for my child if it is considered medically necessary. The responsibility for primary health insurance coverage rests with the participant.
I also grant CORINTHIANS SOCCER ACADEMY LLC the right to use the player's name, images or the like in printed, broadcast and other material related to the Programs provided for such use as a participant in the programs.
I understand that I am signing a contract with CORINTHIANS SOCCER ACADEMY for a period of 11 months starting August 2022 until June 30, 2023. In case of breaking the contract before the established time, I accept that I must pay a fine of $300.00, which must be payed to obtain the release letter
EXCEPTION AND RELEASE
Agreement to participate in the Soccer Program and After School Programs and any location where CORINTHIANS SOCCER ACADEMY LLC offers soccer training sessions.
Participation in all sports can be intrinsically dangerous and, regardless of the care taken to avoid injuries, it is impossible to guarantee the safety of all participating individuals. The game of soccer is no exception. In fact, soccer has many inherent dangers.
It is a sport that demands cardiovascular fitness, coordination, and agility. Players can spend more than 45 minutes in intense physical activity and must be constantly aware of their environment (other players, game and ball itself). Although soccer is not generally considered a contact sport, at competitive levels, it is. Although many risks can be avoided, some simply cannot.
While playing soccer, accidents happen. Some of these accidents can be minor such as collisions with other players, collisions with the ground and being hit by the ball. Other accidents can be more significant, such as collisions with the goal post. Other injuries may also include:
- Minor injuries such as scrapes, bruises, strains, blisters, and sprains.
- More serious injuries such as fractures, contusions, muscle tears and / or ligaments.
- Catastrophic injuries such as heat stroke, cardiac arrest, and death.
To reduce the risk of injury, participants are expected to comply with the following rules:
All participants must be physically prepared before the training begins.
All players must adhere to the rules of soccer, thus creating a safe playing environment.
The registered participant agrees to follow the preceding safety rules, all the common rules of Soccer practice. In addition, the registered participant agrees to report any insecurity in the practices, conditions, or equipment to the instructors of CORINTHIANS SOCCER ACADEMY LLC
I, the undersigned parent / guardian, certify that the registered participant:
- He / she has a sufficient standard of physical fitness to participate in soccer.
Understand that he / she should suspend the activities at any time if he / she feels discomfort or undue stress.
- I have indicated in the medical information any conditions that could affect the ability of my participant to play soccer and will verbally inform the coaches of CORINTHIANS SOCCER ACADEMY LLC immediately.
- I have read the above information, accept, understand, and acknowledge the risks associated with participation in soccer and I am voluntarily accepting the participation of my represented in the activity, assuming all risks inherent in the sport.
- I agree that in the event of a medical emergency, CORINTHIANS SOCCER ACADEMY LLC will call the Medical Emergency Service to provide necessary assistance to my principal and will be financially responsible for any medical
expenses involved.
Disclaimer: In consideration of CORINTHIANS SOCCER ACADEMY LLC allowing my represented to participate in the activities of the Academy, on my behalf, my family, my heirs and my assignees, I hereby release CORINTHIANS SOCCER ACADEMY LLC, its agents and their employees responsible for injury, loss or death of the aforementioned participant while using any facility, equipment or in any way associated with participation in soccer activity now or in the future, as a result of ordinary negligence. I understand that this registration system is provided by CORINTHIANS SOCCER ACADEMY LLC is for participation in the sports activity for the benefit of my represented. I understand that CORINTHIANS SOCCER ACADEMY LLC does not sell, rent or share my information with anyone else.

Mondays-Wednesdays-Fridays
From 6:00pm to 7:30pm
